How it works

Once you've signed up to the Mopheth Pharmacy loyalty Card scheme, you can start collecting points on almost everything you buy. When you get to the till hand your Mopheth Pharmacy loyalty Card to the cashier together with what you are buying. Your points will be instantly added to your Mopheth Pharmacy loyalty Card scheme. You will earn points on purchases made which you can redeem at any of our outlets on the presentation of the Loyalty scheme card.

Terms and Conditions

1.The card

  1. Use of the MOPHETH LOYALTY SCHEME CARD (hereafter referred to as "the Card"), shall constitute your acceptances of these Terms and Conditions
  2. MOPHETH PHARMACY reserves the right to vary these terms and conditions or to substitute or withdraw the loyalty  scheme by writing to customers(by letter or email) and /or by giving notice, which will be displayed on site at all the Mopheth Pharmacy outlets.
  3. Use of your card or any card issued under a scheme replacing this scheme shall constitute your acceptance of the varied and /or new terms &conditions.
  4. Your card is not a debit, credit or a cheque guarantee card, and cannot be used as such.

2. Applying for the card

  1. The Loyalty scheme is open to Nigeria residents of twenty years(20) and above
  2. In order to become a member of the Loyalty Scheme, it is necessary that you submit a fully completed Application Form to any of Mopheth Pharmacy outlets.
  3. Mopheth Pharmacy reserves the right to refuse any application or register a card at its sole discretion.
  4. Once accepted, you will receive a Loyalty Card with a unique number on it.
  5. If at any time your contact changes, it is your responsibility to advise Mopheth Pharmacy of this so that they can update their records.

3. Earning Points

  1. You must present your card at the time of purchase to qualify for any points
  2. The number of points you gain will depend on the product you are purchasing. Please check the points menu provided at Mopheth Pharmacy tills to check how many points you can earn per purchase
  3. Certain products may be excluded from the scheme at the discretion of Mopheth Pharmacy
  4. If you seek a refund in respect of any product purchased, you must present your card so that any point issue at the point of sale can be removed from your account
  5. Mopheth Pharmacy reserves the right to change the rate of awarding points and the type of qualifying purchases upon giving notice in writing and /or as displayed on site in all the Mopheth pharmacy outlets.
  6. You can check your points upon presentation of your card at any of the Mopheth Pharmacy outlets.

4. Redeeming Points

  1. Your cards can only be used at Mopheth Pharmacy outlets
  2. Points can only be redeemed as full payment for items purchased in the pharmacy
  3. The points menu provided at Mopheth Pharmacy outlets outlines how many points  are needed to purchase any item of your choice
  4. Mopheth Pharmacy reserves the right to revise the value of points, how they can be redeemed and the products included in or excluded from the loyalty Scheme
  5. If points have to be redeemed by a certain time ,the management will give notice of this by no less than 28 days

5. Special Promotions

  1. Mopheth Pharmacy may introduce, from time to time, special promotions enabling the Cardholder to collect ‘bonus points’
  2. These promotions will be at the sole discretion of Mopheth Pharmacy and may be time/availability restricted.
  3. Mopheth Pharmacy can set a limit on the number of points awarded to each account in respect of special promotions.
  4. Additional Terms & Conditions to those contained herein may apply.

6. Issuer’s Rights

  1. The card is issued by and remains the property of Mopheth Pharmacy.
  2. Mopheth Pharmacy reserves the right to withdraw the card at any time or refuse to renew or replace the card. Points remaining on withdrawn cards will be lost.
  3. Reasons for withdrawal or non-replacement of cards may include (but are not restricted to) – any abuse or attempted abuse of the Loyalty Scheme; any use or attempted use of a card or points in a manner which is contrary to the Terms & Conditions; any reasonable suspicion of dishonesty on the part of the Cardholder, in connection with the Scheme; any reason that the Cardholder has been barred from Mopheth Pharmacy.
  4. You must surrender the card to Mopheth Pharmacy upon request.
  5. If you have not used your card for at least 12 months, Mopheth Pharmacy reserves the right to cancel the card, with any points balance lost.

7. Cancelling a card and/or reporting it lost or stolen

  1. Cardholders are responsible for the safekeeping of their own Cards.
  2. Mopheth Pharmacy accepts no liability for the loss of cards or points.
  3. If your card is lost or stolen, please contact Mopheth Pharmacy on 0700MOPHETH We will place a stop on your old card and issue a new one. Any points used prior to the stop being placed will be lost. (Please note, there will be a charge of for a replacement card.)
  4. You may cancel your card at any time. Please return it to us at: Mopheth Pharmacy 23A Adeola Odeku, Victoria Island, Lagos

8. Data Protection

  1. Personal data supplied by an applicant or a Cardholder will be held and processed by computer or otherwise by Mopheth Pharmacy to operate the Cardholder’s account; to confirm, update and enhance Mopheth Pharmacy customer records; for statistical analysis; and to establish any Cardholder’s identity.
  2. In each case, the processing may continue after the Agreement has ended.
  3. Where a Cardholder has provided an email address, Mopheth Pharmacy may disclose data relating to an applicant or a Cardholder to any agent or sub-contractor of Mopheth Pharmacy; to any person to whom Mopheth Pharmacy proposes to transfer any of its rights and/or duties under this Agreement; to any organisation that endorses the card.
  4. Mopheth Pharmacy may use information about any applicant or Cardholder to send the applicant or Cardholder information about other goods, services and/or promotions offered by Mopheth Pharmacy, which Mopheth Pharmacy believes may be of interest to the applicant or Cardholder.
  5. Mopheth Pharmacy may contact Cardholders using any of the contact details provided on the Application Form.
  6. Mopheth Pharmacy may give your contact information to selected third parties for them to inform you of special offers, products and services which may be of interest to you.
  7. Any Cardholder that does not wish to receive this information should write to Mopheth Pharmacy 23A Adeola Odeku, Victoria Island, Lagos or tick the relevant box on the application form.

9. General

  1. These Terms & Conditions shall be governed by Nigerian Law and you submit to the exclusive jurisdiction of the Nigerian courts to settle any dispute arising out of or in connection with them, the Loyalty Card or the Loyalty Scheme.
  2. The promoter of the Scheme is Mopheth Pharmacy registered in Nigeria Company Number 232984
